The MedCline Reflux Relief System is not really a pillow — it is a sleep position device. A large inclined wedge with a patented arm pocket, paired with a body pillow, holds you on your left side at roughly a 20 to 30 degree incline and keeps you there. That combination, rather than any material, is the product.
The reasoning is anatomical: on your left side, the stomach sits below the junction with the oesophagus, so acid has to fight gravity to come up. Add an incline and it has to fight harder. Wedge-only setups tend to fail because people slide down or roll off; the arm pocket is what stops that.

Yes, if night-time acid reflux or GERD is waking you up and a stack of pillows hasn't helped. It is expensive, bulky and only works on your left side — but no ordinary pillow does what it does.

What the arm pocket solves
Side sleeping on a wedge normally means your bottom arm is either trapped under your ribs or thrown over your head. The MedCline wedge has a channel through it that your bottom arm slides into, which drops your shoulder into a neutral position and locks your torso in place.
The practical effect is that you stay put. The wedge-and-pocket setup is designed to hold you on your left side all night, which is the mechanism behind MedCline's clinical-study claims — a plain wedge typically loses the position within a couple of hours.
Getting the size right
Sizing is by torso length, not height, and it matters more than anything else here. Too short and your shoulder sits above the arm pocket; too long and your neck loses support at the top of the wedge. Measure from the top of your shoulder to the top of your hip before ordering.
Comfort and the adjustment period
The first two or three nights feel strange — the incline is more aggressive than a stack of pillows and the body pillow means you are effectively sleeping inside a structure rather than on a mattress. Most people adapt within a week.
It is firm by necessity. A soft wedge would sag and lose the incline, so expect supportive rather than plush, and expect it to run warm because your torso is against foam on two sides.
Who it helps beyond reflux
Because it holds an incline and a fixed side position, it also helps some people with post-nasal drip, mild positional snoring, and shoulder pain aggravated by lying flat on the joint. It is CPAP-compatible in the sense that the head is elevated and clear, though hose routing takes a night to work out.

Final verdict
MedCline is a medical-grade solution rather than a comfort upgrade, and judged on that job it works better than any pillow stack we've tried. A 9.0/10 — measure your torso carefully, and be honest with yourself about whether you can sleep on your left side.
